Thank you for helping us capture professional headshots for the Matilda cast. If you would like to receive a digital download of your student’s headshot or order printed copies, please complete the short model release form. If the form is not completed, your student’s photo may still appear in the school display but cannot be shared with you digitally.

Headshots will be taken during rehearsal on April 7th and April 9th at Mount View Middle School.

Participation in the headshots is optional; however, students who opt out will not appear in the display. If you wish to opt out, please notify Mr. Young before April 7th.

Please complete the form below to agree to the terms of the model release.

  • If you are 18 years or older, you may sign for yourself.

  • If you are signing on behalf of a minor child, you must be the parent or legal guardian. By submitting this form, you confirm that you are authorized to do so.

For good and valuable consideration, the receipt of which is hereby acknowledged, I irrevocably authorize Michael Lamb (Photographer) to use photographs of me (or my minor child) and/or my (or their) property. I grant him, his assignees, licensees, legal representatives, and transferees the right to use and publish (with or without my or my child's name, or with a fictitious name) photographs, pictures, portraits, or images described below in any and all forms and media. This includes, but is not limited to, composite images, distorted representations, publicity, illustration, commercial art, advertising, publishing (including electronic publishing on websites or other digital platforms), and any lawful purposes as determined by the Photographer or studio.

I waive any and all rights to review or approve the use of the images, any written copy, or the finished product. I confirm that I am of full legal age and have read and fully understand the terms of this release.
