What you need to do: Scroll thru this page, fill out the waiver, specify if you want prints(optional), and click on submit at the bottom of the page. To have your student’s headshot print included in the display for Matilda, a signed release is not required.
About the Headshots
Every cast & production team member will receive a professional headshot at no cost.
One edited digital file is provided free with a signed release. Prints for personal use can be ordered in step 3 below.
8×10” prints of the headshots will be supplied by the photographer and displayed outside the auditorium during the production.
Headshots will be taken during rehearsal on April 7th and 9th at Mount View Middle School.
Parents or students may elect to opt out of headshots, but will not be part of the display. Please let Mr. Young know if you wish to opt out.
I when the digital versions are ready for download will email the gallery link and password to the email address used in the waiver.

What will the headshots look like?
Studio-style portraits of each cast or crew member, consistent with past productions.
What if I don’t sign the release?
Your student will appear in the printed display at Mount View Middle School, but no digital copy will be delivered. You will not be able to order prints for personal use.
How much does it cost?
Nothing! The school receives the prints and each student receives one digital headshot free of charge. Additional prints for personal use are available for purchase.
Where will the photos be used?
Headshots are for parents’ personal use and for Hammond High School to use in promoting this production.

This includes, but is not limited to, composite images, distorted representations, publicity, illustration, commercial art, advertising, publishing (including electronic publishing on websites or other digital platforms), and any lawful purposes as determined by the Photographer or studio.
